Get Up and Go Wellness, LLC may collect the following information:

• Progress and Event Photos participation online or in-person
• Email address, telephone, or mobile numbers for services online or in-person
• Billing Information for services online or in-person
• Par-Q Form-Medical Questionnaire if participating in training and nutrition programs  
• Liability Consent Form to participate in training and nutrition programs  
• Anonymous Surveys to improve website

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
